Why has my RSI payment not been taken from my bank?

I did my 4th declaration online for 2011 on 10th Jan and have the printed confirmation, yet the payment has not been taken from my bank account. Today I received a letter from RSI saying the payment has not been made and that they have added a charge of 12 euros. I started the business in Feb last year and all the other payments have been taken OK. Do I need to send them a cheque or If I have paid correctly could it be a problem at the bank?
