Where do I register as an hommes de touts mains?

My partner is going to register as a hommes de touts mains (petites reparations, petit bricolage). Is it right that he cannot complete the form online because this involves manual activities and/or it is not our main source of income? Does he need to go to our local chamber de metiers?
Do you know what form he needs to take, or will they have the forms there and help him to fill them in? We have been told he just needs to take proof of address and passport - is this all?
