Where do I put the name of the owner on the CFE form?

I can’t work out where I’m supposed to put the name of the owner, nor how I explain it when I don’t work here, other than to do the paperwork, but out on client sites. You also say to tick box 3 CFE (exoneration for new businesses), but that is for Etablissements de spectacles cinématographiques. Should it be 5, which is various types of Entreprises nouvelles (art 44 sexies/septies/quindecies du CGI)?
