Where do I get my attestation d’affiliation from?

After registering as a profesion liberale auto-entrepreneur back in October, I’m now trying to chase up my carte vitale for myself and the family. I have contacted URSSAF and they say they have passed on my details to RAM. I have telephoned RAM and they don’t seem to have any record of me on file. What do you suggest I do from here? In the meantime how do I go about receiving an attestation d’affiliation - i.e. who does this come from?
