Where and who do I pay my declaration to by cheque?

I filed my first declaration and I was surprised to see money was not withdrawn from my account in order to pay my taxes. I just received an Avis Amiable reminding me I owed money, even though I thought I had set up my bank account so they could withdraw it directly. The problem seems to be I did not send in the piece of paper to the URSSAF allowing them to withdraw from my account. I now understand I must do that. As to not get in trouble, where and who do I pay by cheque? is it the same address where I was supposed to have sent the withdrawal access format?
