What tax forms do I need to complete my year end return?

I’m still very confused about exactly what forms I need to fill in and send for my end of the year tax return. I managed to register my business and do all my 3-monthly declarations, paid my cotisans and my book-keeping is all in order, but the end of year tax return forms have blown my mind.

I am an auto-entrepreneur, artisan business, I groom dogs and don’t have any other income, savings, don’t own property, etc, so the only thing I need to declare is the money that I have already declared on my 3 monthly declarations. I am single and live in my parents house - they are still in the UK, so mine is the only income at this household at present.

Please can you tell me exactly which forms I need to complete to make my end of year return?
