What month is déclaration mensuelle de chiffre d’affaires for?

I have received two similar forms advising me of my tax rates and asking me to complete and send in by 31 January 2013. I registered as an auto-entrepreneur business in France in September. When I complete the form, what month is it for? If for September, will I always pay 3-4 months in arrears? Yet the small text suggests that I must declare cotisations and impots before the last day of the following month, but that would mean I must do December’s income in January, so what do I do about September - November?

Also, it states I have 4 different codes:

631 ventes de marchandises
646 pretations de services commerciales ou artisanales
685 autres prestations de service
576 formation prof. commercant

I will declare sales of online MP3s under 631, but I’m not sure where to declare advertising revenue from my website.
