What is French business form 2042 C for?

I received a letter from URSSAF informing me that I have to declare my earning with a declaration complémentaire de revenus 2042 professions non salariés. I am having trouble finding this form on the imports.gouv.fr site. I am also confused because the form from URSSAF has the Montant des recettes pre-completed but leaves the box A reporter sur votre declaration complémentaire de revenus 2042 C blank. Could you please advise how I can find the correct form and how it should be filled out?
