What is declaration social des independents form for?

I recently paid my two trimestrial payments for October 2013 to Jan 2014 and Jan 2014 to March 2014. Now I’ve found another form from the RSI called declaration social des independents. I don’t understand what I have to do with it, what I should complete on the back and where. I am an auto-entrepreneur 85.59B (Autres enseignements - Sundry education).
