What is an Attestation sur L’Honneur?

I am almost there with changing over my health affiliation from CPAM to RSI - however, RSI have asked me to send an attestation sur l’honneur for my husband who is my beneficiary. They haven’t sent me a form to complete - is there a form or do I have to write a letter and if so, what should it say?
