What do I do with form Attestation Fiscale 2011?

I’ve received a letter titled Attestation Fiscale 2011 but I’m not 100% sure what I’m supposed to do with this. It lists my already declared turnover for 2011- except the last one due end of this month. I’ve read that it is used when completing the impot sur revenu form but I just wanted to check if I need to do anything with this form and if so what?
