VAT Threshold Questions for Auto Entrepreneur

I apologize if this has already been covered, but I thought I saw conflicting information from the Manage Your Auto-entrepreneur series with regards to charging or not charging VAT your first year.

Specifically for my wife, who is an online fitness instructor whose clients are based mostly out of the U.S.:
1.) What is the VAT threshold?
2.) When should she register for it?
3.) Does she charge it to all customers or just those based in the EU/France?

Any other recommendations/tips for her specific scenario would be appreciated.
