Understanding the Tax Professionelle Form

I started self employment on the auto-entrepreneur on the 1st Feb 2009 and have paid up until now, 3 quarterly, on line cotisations. The full year will be complete after Jan 31st. I have been sent a Tax Proffessionelle (notice por remplir la declaration Provisoire) form. Does this form have to be filled in now, or after my full business year ending, in 2 weeks time? I am finding the form quite difficult to understand. Could you enlighten me, or tell me if there is an English translation of the contents? Would the Tax office help me to fill it in, if I went down to the office and also what documents should I take with me?
