Transfer of health care file from CPAM to new organisme santé?

If one becomes self employed in France from previously being an employee, that means transfer of health care file from Caisse primaire d’assurance maladie (CPAM) to new organisme santé (related to régime social des indépendants (RSI) or (Unions de Recouvrement des Cotisations de Sécurité Sociale et d’Allocations Familiales (URSSAF).

Does this mean new social security number and new carte vitale reissued by new organisme sante or the one created by CPAM before stays active, just contains different information?
