Successfully registered as auto-entrepreneur: how to get carte vitale and pay charge sociales?

Thank you for your excellent advice on getting started with the auto-entrepreneur system. I bit the bullet and completed the online registration form on 17th January, and today received a letter dated 20th January containing my SIRET and SIREN numbers, all set up. So now I am wondering, what do I have to do in order to get my Carte Vitale etc? There are also no instructions on how to make my declarations/payments, will they follow under separate cover do you think?
