Registration of micro entrepreneur with CCI - Ajout RCS

I am completing the paperwork (form 15260*01) to register my micro-entreprise with the Chamber of Commerce, and have found a list of documents that needs to be sent with the form but am not sure what some of them are. Could you please advise:

What do I provide for ‘Imprime fiscal’?
What do I provide for ‘choix de l’organisme conventionne’? I am already listed on my husband’s mutuelle (which he has through his employer). Can I just provide an attestation for that mutuelle or do I need to get separate insurance for my micro-entreprise? I already have a carte vitale.
if I have to get this separate insurance for the micro-entreprise, does that mean I am double insured and can remove myself from my husband’s mutuelle?
what do I provide as ‘autorisation du proprietaire’? I am working from my own home.
what do I provide as ‘pouvoir’?
