Registering mix up?

I am Jo Anne, the wife of Brian, and I filled out the paperwork online to set him up as an auto-entrepreneur. The forms were mailed to us and we then sent a copy of his passport to Paris with the forms. That was in March and we have not heard anything. However, 24.80 has been taken out of our account for 3 months now (12/03/2012 CB AUTO ENTREPRENEU 10/03, PARIS 75, CB N° 1401425386). This is copied from our bank statement. Also URSSAF contacts us each month to say we did not report our employee’s earning. The Declaration de debut d’activite number is U7901198xxxx.
