Registering and declaring cotisations online?

I have recently registered a new business in France and have started receiving paperwork for it. RSI (régime social des indépendants) has advised me to register for an online account to pay my cotisations. The registration process asks for Nom as one of the identifiers but I notice that on all paperwork my Nom is given as my maiden name.

Should I use my maiden name for the account registration or can I use my married name?
Do you have anything which explains/shows how to fill in the déclaration trimestrielle form?
