RAM has closed online account, what can I do now to access account?

I set up an auto-entrepreneur business here in France in March 2016. RAM gave me a provisional number d’immatriculation / social security number and stated that in September 2016 I would have to re register using my carte vitale number. I still have no carte vitale card and I am locked out of my RAM account and I cannot use the RSI online services without it either.

  1. How can I access my two accounts without a carte vitale?
  2. How do I chase up my carte vitale?
