Problem adding bank details on Net Enterprise?

I am trying to register my new bank details on Net Enterprise and I’ve added them but there is nowhere for me to print off the details to give to my bank. I deleted my old details and inputted the new ones. I have this message on the page:
En cas de modification, n’oubliez pas de renvoyer par voie postale à l’adresse indiquée le formulaire d’adhésion au télérèglement dûment complété et signé, sans rature ni commentaire. Ce formulaire (format PDF) vous sera présenté dans une nouvelle fenêtre en cliquant sur Afficher le formulaire d’adhésion.
But there is nothing to click, can you help me?
