Paper form to register as an auto-entrepreneur?

I have tried on multiple occasions to submit the online auto-entrepreneur business form in France, however after pressing validate it does not submit. My wife is from outside the European Union (EU) and on other posts it mentions there are issues / bug with the form when trying to add someone from outside the EU. Where can I find a download for the most recent paper form to fill in?
