Online Declarations & Company Name

In making tax declarations online via lautroentrepreneur.fr, I’m met with different categories than in the letter I received (I am a teacher and consultant). In my letter from SSI there is a teacher option ‘formation prof. obligatoire’ as well as ‘autres prestations de services accre’. Only the latter is available online, so should I declare all turnover as ‘autres prestations de services accre’? Or is there another way to go about this?
Also, a silly question, but how do I find out my company name? In registering I believe I just used my name (there is no ‘nom d’usage’ on my registration certificate), so when I confirm my payment details, I assume I just use my name when it asks for company name?
