Micro-enterprise registration and health choice with Guichet Entreprise?

I am currently trying to register a business as a micro enterprise in France through guichet-enterprisses.fr I am a little confused on a couple of things.

  1. Under regime fiscal where it asks Cochez si vous souhaitez opter pour le versement libératoire what is this referring to?
  2. Where it talks about Quel est votre régime d’assurance maladie actuel?? and regime general, agricole and so on
  3. Where it asks which accredited body I use for health insurance. I assume this means I have to take out health insurance before I can complete this?
  4. Who do they mean by the recipient? The local official in the Chamber of Commerce?
