Making my first declaration - is it to the right organisation?

I am a profession liberales - an administrator working from home. I’ve received my first declaration form from RSI but having watched your video about making the first return it looks as if I should be making my declaration to URSSAF. As the form has come from RSI is it correct that I should be making the return to them? The % tax amount pre-filled on the form is 21.3% - is that correct? I started my business on 1st Jan this year; the form is titled 1er trimestre 2010 and the ‘date limite di’envoi’ on the form is 2nd August - this seems late but very nice. Is this also correct.
