l’Assurance Maladie Paris – spam or legitimate?

I checked the forum but couldn’t find any other questions about this so I hope you can give me some advice on this.

I received a letter from l’Assurance Maladie, but I’m uncertain about whether it is a scam or legitimate. It asks for a lot of personal data so I wondered if I could run it by you first?
