How to send a Feuille de Soins to get reimbursed

When we send in the Feuille de Soins do we send the hardcopies, or is it ok to send photocopies of them? I’m concerned if they lost I would like to keep the hard copies if possible.
Also is it required to send them with a cover letter? Do I send all of them in the same envelope or should I separate by type (from a doctor vs from a pharmacie / prescription).
