How to fill in declaration revenus 2042 C?

I am an auto entrepreneur. Where do I declare my auto entrepreneur earnings on the Declaration Revenus? I opted to pay tax and social each quarter. I have received an Attestation Fiscale Regime Auto-Entrepreneur which states:

A reporter sur votre declaration complementaire de revenus 2042C
Cases 5TE, 5UE et 5VE (cadre D)

However, I’m confused about where to enter this income on the 4 page Revenus 2012 declaration.
