How do I use declaration mensuelle de chiffre d’affaires?

I registered as an auto-entrepreneur in France in October last year and submitted by first three month tax return for the 29 February in March. In April I received three more forms to complete they were all dated 2 May 2016. Do I fill one in for 2 March, 2 April and 2 May, or just complete one for the three months up to May from 29 February? I have today received a form to complete for 31 May. What should I do?
