How do I get a new attestation and get my carte vitale updated?

I was registered with the CPAM (I was salaried in UK) and had a Carte Vitale. My rights under that finished on 31st December 09 and I started as an Auto-entrepreneur on 1st January. My business is registered as a profession liberales.

I haven’t received any communication regarding health cover so am currently having to pay all of my medical charges in full. I’ve written to RSI who don’t have any record of me and RAM Professionalle who I’ve chosen to collect my social charges. They have no record of me. I’m wondering if it is URSSAF that I should be registered with. Please could you tell me how I can get a new attestation and get my carte vitale updated.
