How do I fill out the URSSAF form for auto-entrepreneurs?

I have received my URSSAF form for my business in France and there are two codes printed on the form:

  1. 688 Activities relevant de la CIPAV Taux en 23.3%
  2. 578 Formation Prof Activitie Liberale Taux en 0.2%

My business is registered as a yoga teacher and I’m not sure under which code I enter my total earnings?
