How do I fill out cotisation fonciere des entreprises 2015?

I registered as an auto-entrepreneur in August 2015 and am filling out the Cotisation Fonciere des Entreprises. I found your tutorial for how to fill out the 2013 form, but I think the sections are slightly different this year.

Section A1: My form came pre-filled out with my activites excercees as Enseignement Secondaire Classique, but I am an art historian and writer - is this the correct description?
Section C: Biens du nouvel établissement passibles d’une tax foncière. How do I fill this out as someone who works from home, not even really at a home office, but at my kitchen table?
Finally, I seem to have received two of these forms. Do I complete and return them both? Where do I send them?
