How do I fill in the Trimestrielle form?

I’ve been studying the Declaration Trimestrielle de Chiffre D’Affaires and I can see what I need to complete in the first part of the form, but about two thirds of the way down it says Changement de regime D’Imposition, underneath that it says Vous relevez d’un regime reel d’imposition depuis le: followed by a box in which to put the date. How do I complete that bit? Also, do all the figures I put in, the amount earned, and the cotisations I pay, need to be rounded up?
