How do I fill in the Cotisation Fonciere des Enterprises form?

I have received the above form and, after having read your blog and other posts on this subject, am still a little unclear as to how proceed. My auto-entrepreneur business is set up to pay income tax at source (I get a quarterly bill), therefore: Do I have to fill the form in and send it back? If so, is there a guide anywhere on how to fill it in?
