How do I fill in tax form 2042C?

I have downloaded form 2042C and I am following the advice from Valerie as per her article. As I haven’t yet paid the tax on my earnings, I have put our sales in the 5KO box. On page 2 it asks for information regarding siret number and other information. Do I complete parts of this? Then on page 3 it asks about Régime micro entreprise and mentions Chambre d’hotes - which is what I do. Do I complete more boxes (5NO) with the same recettes as we put in 5KO? I lose our phone line/internet tomorrow so a speedy reply would really be helpful.
