How do I combine the two different occupations on the Auto-entrepreneur form?

Still haven’t filed my application after about 2 months of thinking about it. Now need to do something quickly. I am a teacher and have been teaching for an association and Le Greta (education nationale) since 2003 and 2004 respectively. I am going to file an application as auto-entrepreneur under the services section (welcoming and supporting guests in holiday rentals - 96.09 autres services personnels as you recommended) but would also like to add teaching to my auto-entrepreneur application as I am resigning from the work with the association andI think that a number of students will want to stay with me. How do I combine the two on the auto-entrepreneur form?
