How do I change from carte vitale via CPAM to RSI?

My wife and I are approaching the cut off date for our cover through CPAM (E106). I registered as an AE in Feb 2010 but as yet have not started my business and have declared a nil income. I will be working from early January. Can I get the carte vitale before I have paid social charges? I haven’t heard from the company I chose on the original application. Should I contact them or RSI?
