Getting a carte vitale for an auto-entrepreneur business setup in France?

As an auto-entrepreneur business setup in France we are still trying to get a carte vitale health card. We have been asked to provide a certified copy with seal of a translated birth certificate and marriage certificate. Do we have to do this through a registered nominated translator or can you do this in which case can you gives me a cost to do this?
