Form 751-SD questionnaire relatif a l’activite professionnelle

My partner and I are both registered individually as agents commerciaux under auto-entrepreneur business scheme in France. Having recently moved we registered new address at Greffe, new Kbis certificates received and now received letter each re Transfert de votre activite professionnelle from the Service des Impots des Entreprises with une demande de renseignments relative aux locaux que vous occupez pour les besoins de votre activite professionnelle (Form 751-SD). We will be working from home. Please could you clarify following?

  1. Identification de votre entreprise. Date de cloture du premier exercice. We originally registered ourselves on 1 December 2014 and moved to our new address on 14 January 2015 so do we put the latter as the date de cloture du premier exercice with perhaps clarification in section 5 asking for additional info or is this not relevant?
  2. Votre comptable. Is this only relevant if we have an accountant, which we do not?
  3. As we are working from home I assume we only fill in the last category: vous exercez votre activite a votre domicile et vous n’avez pas d’autre local a votre disposition and give the square meterage for the business (desk space)

I think that’s all we fill in but some reassurance would be appreciated.
