DSI for EURL (Cerfa 10020*21)?

I am confused about the declaration and the explanation from my accountant I have a mix:

Salary from an employment in the beginning of the year.
Company account of the entreprise unipersonnelle à responsabilité limitée (EURL) from which I have transferred amounts to my private account from time to time.

How should I declare on the form (assuming it is in section 2 only)?
