I have submitted paperwork for my carte vitale, but have now received a follow-up, repeated request. In an attempt to fulfill the original request, I sent a copy of my bank statement and the original statement of my social security number, but they seem to need a document that combines both of these, but I can’t visualise what this would be. The exact wording of the request is:
l’original d’un releve d’identite bancaire, postal ou de Caisse d’Epargne mentionnant le nom, prenom, no de securite sociale.
Can you tell exactly what type of document they need?
