Do I need to complete (and what) AG2R form?

I’ve got a letter from AG2R: Appel de cotisations annuel 2014. As I understand it’s about my employees. I’m registered as auto-entrepreneur (code APE 8121Z) and have no employees. What do I need to complete in this form, or do I ignore it?
