Do I have to fill in the CFE form 1447-C-K?

I have received a form 1447-C-K to complete for cotisation fonciere des entreprises (CFE) tax. I understood that I would not have to pay this until December 2015. I am an auto-entrepreneur with a chambre d’hotes and gite and my business start date was 1 June 2014.
- Why do I need to complete this now?
- Is there a maximum amount that I will be liable for just so I have an idea?
- Is there a time limit for completing the form?
- Also, there seem to be two copies of the form and it has been sent to me in my maiden name, not my married name, is this normal?
