Declaration for 2eme and 3eme trimestrialle on same form?

I am in the process of completing my declaration that is to be submitted by 31 October. The form states declaration 2e Trimestre 2014, however on the small print on the back it states:

Vous créez votre activitéau 1er avril et vous opter pour un déclaration et un versement trimestriels de vos cotisations vous effectuerez pour le 31octobre de 2 déclarations survivantes 2e and 3e trimestre.

Does this mean I add the two trimestres together and declare on this one form?
