Declaration de debut d’activite?

I declared myself as auto-entrepreneur in February and I received my SIRET number a few days later. The date de prise d’activité is 15/3/2010. I went to set up a bank account today for all things AE and they are asking for a form called declaration de debut d’activite, PO that is to be provided by the CFE.

They have opened my bank account but are holding my bank card until I can provide this document. I’ve looked through all of my papers, I don’t see anything like that. Was I supposed to officially declare that I’ve started working through the auto-entrepreneur? I thought I did that when I did the teledeclaration? Also, I haven’t heard from the insurance or carte vitale? Is it too early to follow up?
