Déclaration de Choix de Médecin Traitant from CERFA

My husband Robert is a new auto-entrepreneur business here on France and on his P0 application form (to URSSAF) he opted for RAM assurance maladie, although we both have a Carte Vitale and are registered with CPAM. Being asked to make this choice was confusing, but it’s done.

He received a Déclaration de Choix de Médecin Traitant from CERFA which has to be signed and stamped by his treating physician. We have been registered with the same doctor since 2007, do we still need to complete this form? If so, who is considered l’assuré and who is considered the bénéficiaire? This sounds like an odd question to ask, but with CPAM, I am the assured and Robert is the beneficiary. A booklet came with the form, and one of the chapters is about Cotisations d’assurance maladie. This looks like my husband will be paying cotisations to RAM, not URSSAF, based on his auto-entrepreneur income, is that right? And is he exempt for the first two years?
