declaration de chiffre d’affaires form

I received a form called “Declaration de Chiffre d’affaires du 2e trimestre 2021” today.
What is this form? Should I declare my revenue for May-June?

But I asked you about this before: my company officially starts on May 13, and if I wait for 90 days, then my first declaration is on Aug 10 or in the middle of the quarter, so my first declaration supposed to be in October, right?
