Completing the first declaration trimestrielle for RSI?

I’m just about to fill in the form I have received from RSI having started as an AE on 23.01.12. I have seen your helpful video, but just a quick clarification please. My form has been pre-filled with three Code de Gestion’s numbered 508, 518, 572, relating to the three activities - merchandises, presentations and Formation Professionnelle at the Taux. With rates of 12.00, 23.10 and 0.30 respectively. I only have services, so I can fill that in, but what do I do with the Formation category?
