Closing down auto-entrepreneur and move back to CPAM from RSI?

I want to close down as an auto-entrepeneur, and move back to CPAM from RSI/RAM, what is the correct way of doing this? I paid my first 2012, 3 monthly trimestrielle in April, I have earnings to declare up to end May, but want to leave auto-entrepreneur as soon as possible. Can you advise best way forward please.
