Can you help with change of address form P2 PL?

We have been advised to fill out a P2 PL form by the Greffe to advise a change of address. A bit unclear on which boxes refer to just a change of address. Could confirm/answer following queries?

Cadre 1. Which box to tick? Lieu d’exercice ou etablissement and then modification or Transfert or Autre modification?
Cadre 10. Which box to tick? Transfert or Autre modification?
Cadre 11. Fill in old address, but which box to tick on right hand side or leave blank?

Would be great help if you could help with these, have been advised to return with a proof of address.
