Auto entrepreneur registration form for general builder?

My husband is re registering his building business in France as an auto entrepreneur. He closed the last one some time ago. He is a general builder, so what should he go under in the choices from the two drop down menus? Also, what do I tick in the conjoint section, do I tick the box demande d’accre? What is that? Do I tick entreprise individuelle and if so, what is the extra form about, declaration patrimony? What do I choose for regime assurance maladies actuel and who is best to go with? Previously we were with Previfrance, but is there a better one? Is it best to go trimestral or mensuel? Finally, what is the option fiscale?
